Walking away from Diabetes
 
This is a new programme that is offered to residents of Preston, Chorley or South Ribble.
 
Residents must be aged 16 or over and must be “at risk” of developing Type II Diabetes.
 
For patients to work out if they are at risk, they should follow the instructions on the Diabetes Risk Score Tool (attached).

If a patient score’s 25 + then they may be at risk and Walking Away From Diabetes could be the programme for them.
 
There are a number of ways patients can be referred onto the programme.
 
  • Self-refer directly either by completing an online Health Questionnaire form, by calling 01772 906518 or by emailing Francesca Webster at f.webster@preston.gov.uk . Courses are due to commence in December 2013.
  • local GP or practice nurse can also refer patients using the same details as above for a risk assessment.
 
When: Courses are delivered in the morning, afternoon and/or evenings.
