Summary care records

A Summary Care Record (SCR) is an electronic summary of important information from your GP medical record. It allows authorised health and care professionals involved in your care to access important information when you are receiving care away from St Fillans Medical Centre.

Your Summary Care Record contains information about:

  • medicines you are taking
  • allergies
  • any previous reactions to medicines.

It may also contain additional information, such as significant medical history, long-term health conditions, immunisations, care preferences and communication needs.

Having this information available can help healthcare professionals provide you with safer care, particularly if you need treatment when the practice is closed, in an emergency or when receiving care elsewhere.

 

Who can see my summary care record?

Your Summary Care Record can only be accessed by authorised health and care professionals who need the information to provide you with care.

Healthcare professionals should ask for your permission before viewing your Summary Care Record. In some circumstances, such as an emergency where you are unable to give permission, access may be made in your best interests.

 

Can I opt out?

Yes. You can choose:

  1. to have a Summary Care Record containing Core and Additional Information
  2. to have a Summary Care Record containing Core information only
  3. not to have a Summary Care Record.
