Request a sick (fit) note
If you’ve been off sick for 7 days or less
We do not issue fit notes for periods of sickness of 7 days or less, you can self-certify you absence directly with your employer

If you’ve been sick for longer than 7 days
If you have been unwell for more than 7 days and need a fit note, please submit an online request explaining why you need one and the dates it is required for. Fit notes cannot be issued in advance and can only cover a period when you have been assessed as unfit for work. You do not usually need a GP appointment just to request a fit note; your request will be reviewed by the appropriate member of our team, who will advise you if an assessment is needed.
